How Does EPC Work?
An Enhanced Primary Care (EPC) Care Plan is a government-funded program designed to provide eligible patients with access to essential allied health services, including physiotherapy. It aims to promote improved patient outcomes and overall well-being by enabling individuals to access the care they need.
What is an EPC Care Plan?
- Speak to Your GP: The first step in the process is to consult with your General Practitioner (GP). If your GP identifies that you have a chronic condition, they can assess your eligibility for an EPC Care Plan.
- Get a Referral: Once your eligibility is confirmed, your GP will provide you with a referral to Braybrook Physiotherapy. This referral allows you to access physiotherapy services under the EPC program.
- Visit Braybrook Physio: You can then schedule an appointment with our experienced physiotherapists at Braybrook Physio. During your initial consultation, we will work closely with you to assess your condition and develop a tailored treatment plan.
- Personalised Care: Our dedicated team of physiotherapists will create a personalised treatment program that focuses on managing your specific condition. We aim to reduce pain, improve your mobility, and enhance your overall well-being.
- Regular Reviews: Your progress is essential to us. We will conduct regular reviews to ensure that your treatment plan is working effectively. If necessary, adjustments will be made to optimise your care.
Billing and Payment: Under the EPC program, you may be eligible for up to five Medicare-rebated sessions per calendar year. Our friendly staff will assist you in managing the billing and payment process.
